Complete this form if you are registered student wishing to transfer to either:

• the register of student dispensing opticians
• the register of student optometrists

If you are not on a student register but have been on one previously and want to:

• re-join the same register then complete a Student Restoration Form
• join the other student register, then complete a Student Registration Form
